Understanding PMS
Premenstrual syndrome, commonly known as PMS, describes physical and emotional symptoms that occur in the week or two before a period and usually improve shortly after the period begins.
PMS can feel different for every person. Symptoms may also change from one menstrual cycle to another, although they generally follow a recognisable monthly pattern.
Common emotional and cognitive symptoms may include:
Irritability or feeling less able to cope
Mood swings or increased emotional sensitivity
Anxiety, tension or feeling overwhelmed
Sadness or a lower mood
Difficulty concentrating or mental fog
Changes in sleep
Food cravings or changes in appetite
Reduced confidence or motivation
Common physical symptoms may include:
Abdominal bloating or fluid retention
Breast swelling or tenderness
Pelvic cramping
Fatigue or low energy
Acne or other skin changes
Muscle or joint aches
Constipation, diarrhoea or nausea
Having an occasional symptom before a period is common. However, it is worth seeking professional support when symptoms repeatedly interfere with your comfort, relationships, work, study, sleep or quality of life.
What Is PMDD?
Premenstrual dysphoric disorder, or PMDD, is a severe premenstrual condition involving intense emotional and psychological symptoms. It is not simply “being moody” before a period.
PMDD symptoms generally appear during the one or two weeks before menstruation and improve after the period begins. They can significantly affect a person’s work, relationships, family life and sense of wellbeing.
Symptoms may include:
Severe irritability, anger or conflict with others
Marked anxiety, tension or feeling on edge
Sudden or intense mood changes
Depression, hopelessness or feelings of worthlessness
Feeling overwhelmed or out of control
Loss of interest in usual activities
Difficulty concentrating
Significant changes in sleep or appetite
Low energy and fatigue
Physical PMS symptoms such as bloating, breast tenderness, headaches or body pain
PMDD needs appropriate medical assessment. It can overlap with or be mistaken for depression, anxiety, bipolar disorder, thyroid problems and other health conditions. Acupuncture and Chinese herbal medicine should be considered complementary care and should not replace diagnosis or treatment from your GP, gynaecologist, psychiatrist or psychologist.
Tracking Your Symptoms Can Be an Important First Step
Because PMS and PMDD symptoms follow the menstrual cycle, recording your symptoms every day for at least two cycles can help reveal a pattern.
You may wish to record:
The first day of each period
When emotional and physical symptoms begin
The severity of each symptom
When symptoms improve
Effects on work, study, relationships and daily activities
Sleep, energy, pain and appetite
Medicines, supplements and hormonal contraception
This information can support your consultation with Julie and may also help your doctor distinguish PMDD from a mental health condition or another medical concern.
A Caring and Holistic Approach to PMS and PMDD
At Essential Acupuncture Clinic, you will be cared for as a whole person—not treated as a collection of isolated symptoms.
Two women may both experience irritability and bloating before their periods, yet the circumstances surrounding those symptoms may be very different. One may also struggle with poor sleep, headaches and high stress, while another experiences pelvic pain, digestive changes and exhaustion.
Julie considers the timing, pattern and severity of your symptoms together with your menstrual history, emotional wellbeing, general health and lifestyle. Your treatment is then thoughtfully tailored to your individual needs.
The aim is to offer a calm, supportive space where you feel heard and respected while receiving personalised complementary care.
How Acupuncture May Support PMS & PMDD Symptoms
Acupuncture involves the insertion of very fine, sterile needles into carefully selected points on the body. There is no single set of acupuncture points used for everyone with PMS or PMDD.
Your treatment may take into account:
Whether emotional or physical symptoms are most disruptive
The stage of your cycle when symptoms begin
Pelvic cramping, headaches or muscular tension
Sleep quality and energy
Digestion, appetite and bloating
Cycle length, bleeding and period pain
Perimenopausal changes, where relevant
Current medicines, supplements and health conditions
Many people also find the treatment environment calming and appreciate having dedicated time to pause, rest and reconnect with themselves.
Acupuncture does not “correct” or “balance” reproductive hormones in a guaranteed way, and individual outcomes vary. It may be considered as one part of a broader care plan for premenstrual symptoms.
What Does Scientific Research Say About Acupuncture for PMS and PMDD?
A 2018 Cochrane review identified five randomised controlled trials involving 277 women. The review found limited evidence suggesting that acupuncture or acupressure may improve physical and psychological PMS symptoms compared with sham treatment.
A separate 2014 systematic review examined acupuncture and herbal-medicine trials for PMS and PMDD and reported symptom improvements in some studies.
Chinese Herbal Medicine for PMS & PMDD Support
Where appropriate, Julie may discuss individually prescribed Chinese herbal medicine as part of your care.
There is no single herbal formula suitable for every woman experiencing PMS or PMDD. A prescription is selected according to your symptoms, constitution, medical history and current health needs. Two women with similar pain, for example, may require different approaches if one also experiences poor sleep and anxiety while the other experiences headaches, fatigue or digestive discomfort.
